Active Ingredients:
ipratropium bromide + fenoterol hydrobromide
Therapeutic actions:
Berodual N DA is a combined broncholytic medication. Fenoterol hydrobromide is selective beta2- adrenoceptor agonist. The mechanism of action is connected with activation of interfacing adenylate cyclase receptor, that leads to cyclic adenosine monophosphate formation increase which stimulates calcium pump work. As a result concentration of calcium in myofibrils lowers and bronchial tubes dilation occurs. Fenoterol weakens smooth muscles of bronchial tubes and vessels and counteracts progression of bronchospasmodic reactions caused by influence of histamine, methacholine, cold air and allergens.
Ipratropium bromide is m-cholinoreceptor blocker. It eliminates effectively bronchospasm, retracts gland secretion (including bronchial). Using inhalation introduction it provokes bronchodilatation, caused mainly by local, but not systemic anticholinergic action.
What is it used for?:
Prevention and expected treatment of chronic obstructive respiratory tract disease with inversive bronchospasm:
• bronchial asthma;
• chronic obstructive bronchitis, complicated or uncomplicated by emphysema.
Contraindications and cautions:
• hypertrophic obstructive cardiomyopathy;
tachyarrhythmia;
• pregnancy (I trimester);
• hypersensitivity to the medication components;
• hypersensitivity to atropine-like substances.
Side effects:
Central nervous system: frequently - tremor, nervousness; possible - giddiness,headache; seldom - mental change.
CVS: possible - tachycardia, heart consciousness; seldom (if using high doses) – diastolic pressure lowering, systolic pressure elevation, arrhythmia.
Respiratory system: possible – cough, local irritation; seldom – paradoxical bronchospasm.
Digestive system: frequently – xerostomia; possible – nausea, emesis; seldom – inversive GIT dysmotility.
Visual organs: seldom – inversive adaptation disorder.
Allergic reactions: seldom – exanthema, angioneurotic glosso, labia, face edema, urticaria.
Other: possible - diaphoresis increase, asthenia, convulsions; seldom – inversive uroschesis.
Interactions:
Simultaneous use of others beta-adrenoceptor agonists, incoming into the system anticholinergic drug bloodstream or xanthic derivants (including theophyllin) make side effects enhancement possible.
Significant decrease of Berudal N DA broncholytic action is probable if using beta-blockers simultaneously.
Berodual N DA potentiation is marked if using simultaneously with monoamine oxidase inhibitors and tricyclic antidepressant.
Drugs for the inhalation narcosis, containing halogenated hydrocarbon (including halothan, ethylene trichloride, enflurane) can potentate Berodual N DA effect on cardiovascular system.
