Description
Lanoxin (Digoxin) helps the heart to beat more strongly and regularly. It is used to treat conditions such as congestive heart failure and atrial fibrillation/atrial flutter (types of fast heartbeats).

Therapeutic actions:
Lanoxin (Digoxin) binds to a site on the extracellular aspect of the α-subunit of the Na+/K+ ATPase pump in the membranes of heart cells (myocytes). This causes an increase in the level of sodium ions in the myocytes, which then leads to a rise in the level of calcium ions. The proposed mechanism is the following: inhibition of the Na+/K+ pump leads to increased intracellular Na+ levels, which in turn slows down the extrusion of Ca2+ by the Na+/Ca2+ exchange pump that relies on the high Na+ gradient. This effect causes an increase in the length of Phase 4 and Phase 0 of the cardiac action potential, which when combined with the effects of Digoxin on the parasympathetic nervous system, lead to a decrease in heart rate. Increased amounts of Ca2+ are then stored in the sarcoplasmic reticulum and released by each action potential, which is unchanged by digoxin. This leads to increased contractility of the heart. This is a different mechanism from that of catecholamines. Digoxin also increases vagal activity via its action on the central nervous system, thus decreasing the conduction of electrical impulses through the AV node. This is important for its clinical use in different arrhythmias.
What is it used for?:
• Treatment of mild to moderate heart failure.
• Control of ventricular response rate in patients with chronic atrial fibrillation.
Contraindications and cautions:
• allergy to any ingredient in Lanoxin or other digitalis medicines;
• beriberi heart disease;
• certain types of heart rhythm problems, such as ventricular fibrillation.
Side effects:
Serious side effects: severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); hallucinations; blurred vision, yellow vision, or other vision changes; fast, slow, or irregular heartbeat; mood or mental changes (eg, depression); severe or persistent nausea, vomiting, or stomach pain; confusion; unusual bruising or bleeding; unusual tiredness or weakness.
Less serious side effects: diarrhea; nausea.
Interactions:
• the risk of Lanoxin is side effects, especially on the heart may be increased by amiodarone, amphotericin B, anticholinergics (eg, propantheline), beta-blockers (eg, propranolol), bupivacaine, calcium, certain stimulants (eg, albuterol, amphetamine, pseudoephedrine), cyclosporine, diltiazem, diphenoxylate, diuretics (eg, hydrochlorothiazide, furosemide), indomethacin, itraconazole, macrolides (eg, erythromycin), propafenone, quinidine, quinine, spironolactone, succinylcholine, tetracyclines (eg, doxycycline), thioamines (eg, methimazole), or verapamil;
• Lanoxin is effectiveness may be decreased by acarbose, certain anticancer medicines, cholestyramine, colestipol, kaolin-pectin, metoclopramide, penicillamine, rifampin, sulfasalazine, or thyroid hormones (eg, levothyroxine);
• the actions and side effects of Lanoxin may be increased or decreased by oral aminoglycosides (eg, neomycin);
• Spironolactone may interfere with certain lab tests for digoxin blood levels and give false readings.
