Description
Captopril is used to treat high blood pressure (hypertension), congestive heart failure, kidney problems caused by diabetes, and to improve survival after a heart attack.

Therapeutic actions:
The mechanism of action of CAPOTEN has not yet been fully elucidated. Its beneficial effects in hypertension and heart failure appear to result primarily from suppression of the renin-angiotensin-aldosterone system. However, there is no consistent correlation between renin levels and response to the drug. Renin, an enzyme synthesized by the kidneys, is released into the circulation where it acts on a plasma globulin substrate to produce angiotensin I, a relatively inactive decapeptide. Angiotensin I is then converted by angiotensin converting enzyme (ACE) to angiotensin II, a potent endogenous vasoconstrictor substance. Angiotensin II also stimulates aldosterone secretion from the adrenal cortex, thereby contributing to sodium and fluid retention.
Capoten prevents the conversion of angiotensin I to angiotensin II by inhibition of ACE, a peptidyldipeptide carboxy hydrolase. This inhibition has been demonstrated in both healthy human subjects and in animals by showing that the elevation of blood pressure caused by exogenously administered angiotensin I was attenuated or abolished by captopril. In animal studies, captopril did not alter the pressor responses to a number of other agents, including angiotensin II and norepinephrine, indicating specificity of action.
What is it used for?:
Treating high blood pressure, heart failure, or certain diabetic kidney problems. To improve survival and decrease heart failure in certain patients after a heart attack. It may be used alone or in combination with other medicines.
Contraindications and cautions:
• allergy to any ingredient in this medicine;
• the second or third trimester of pregnancy;
• known severe allergic reaction (eg, rash; hives; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue) to any other ACE inhibitor (eg, enalapril);
• currently taking dextran sulfate.
Side effects:
Serious side effects: severe allergic reactions (rash; hives; itching; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); irregular or slow heartbeat; any signs of infection such as fever or sore throat; difficulty swallowing; fainting; chest pain; yellowing of the skin or eyes.
Less serious side effects: cough; diarrhea; dizziness or lightheadedness when sitting or standing quickly; fatigue; taste changes; fever; itching; joint pain; nausea; headache; vomiting; weakness.
Interactions:
• Allopurinol or dextran sulfate may increase the risk of an allergic reaction (eg, rash; hives; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue) and lightheadedness upon standing;
• The risk of low blood pressure or low blood sodium levels may be increased by diuretics (eg, hydrochlorothiazide, furosemide);
• Capoten is effectiveness may be decreased by nonsteroidal anti-inflammatory drugs (NSAIDs) (eg, ibuprofen, indomethacin);
• Capoten may increase the risk of side effects of lithium or thiopurines (eg, azathioprine);
• Capoten may increase the risk of side effects, including low blood sugar levels (eg, hunger, shakiness or weakness, dizziness, headache, sweating) of oral diabetes medicine (eg, glyburide);
• High blood potassium levels (eg, listlessness, confusion, abnormal skin sensations in the arms or legs, heaviness of limbs, slow or irregular heartbeat, or stopping of the heart) may be provoked by potassium supplements or potassium-sparing diuretics (eg, amiloride).
